    Applying For Disability Benefits

    If you're applying for disability advantages to social security, you have to do it the soonest possible time after becoming disabled. You have three methods in applying for disability benefits. You can apply online, calling on SSA toll-free number, or calling or visiting your local Social Security office.

    In applying for disability advantages for an adult, you're required to complete an application for Social Security Blessings and an Adult Disability Report forms. The report collects all the details about the disabling condition and how it affects the capability to work. You can complete the forms online or you could call the Social Security office to itinerary an appointment so that they can aid you personally, or by phone.

    Online Application for Disability Benefits

For example, you're to sign papers that would permit your doctor send your medical record to social security proving your disability.

    § Names, addresses, and phone numbers of doctors, caseworkers, hospitals, and clinics that took care of you and the dates of your visits.
    § Names and dosages of all the medications you're taking
    § Medical records from your doctor, therapists, hospitals, clinics, and clinics that you already have in your possession
    § Laboratory and test results
    § A summary of where you worked and the kind of work you did
    § Your the bulk recent W-2 form, or if you were self-employed, a copy of your federal tax return.

    For Family Members

    § Social Security numbers and evidence of age for each person applying for disability benefits

    § If you're applying for disability advantages for your spouse, you should have evidence of marriage, including the dates of prior marriages if applicable.
